The original version of the game has .exe file "6kinoko", and two .dat files "6kinoko_a" and "6kinoko_b". The patches all add a third .dat file "6kinoko_c", so if you don't have that yet, you have the original unpatched game.
It shouldn't matter what version of the game you have currently, since you can just replace that with the file versions you want.

I don't remember very well (it was a decade ago when I played), but there were a few instances of interactions with terrain and objects being a bit odd. I also remember with the original version of the game that certain level exits in
Other than that one possible glitch, I suppose most of the fixed bugs are minor. It's just that by 1.03 the game seemed to have enough polish to be as glitch-free as it could be, and it was a little jarring when glitches that I saw in older versions returned in the content update. A small but noticeable example:
If you want to play the version you got, you can maybe switch between the two patches if you do run into something like wrong paths. And we also have Kilga's old LP if we want a reference to the game in its pre-update state.

6-4 and 6-5 were swapped in the map, so the level environment didn't match with what it looked like in the overworld for those two. And the secret exit for 6-5 wasn't there yet.

Should probably also be noted that raocow's opted for a new system of game voting/choosing so that it doesn't take two years in between votes (which I'm guessing will be mentioned in said video?).

There are the Rabi-Ribi DLC fights too! Going back into that game cold just to play a bunch of Extra Stage boss battles would make for quality viewer entertainment if nothing else.

Having played a little of SF: Adventures recently through Dolphin, it does not emulate the best. It's playable, but hits a lot of slowdown, and the audio and cutscene playback is tied to framerate, so if the framerate dips due to poor emulation, you get really scratchy, slow audio. There are also some visual bugs with HUD elements. Maybe it'd be better in a couple years but right now it's not amazing.
