It's been a while since I poked into SMBX, but I'm pretty sure you can call an event on an individual sprite's death. So you'd just need to have that call an event to deactivate a layer later in the level, and that layer would have the thing which rewards you for the pacifist run.

I was curious about how many countries there were total. A guide to the PS2 version says 195, which would mean you only missed one. But I don't know if the remake just brought the same map over or if it was updated in any way, what with political boundaries being inherently volatile.

In general I'd recommend looking at the katamari. This level in particular could be harder to follow because you need to see the collected objects to know if the thing he just picked up is a twin, but for the rest, there's no critical info. It's worth noting that the game was originally made on the PS2, where the analog sticks are symmetrically placed.
