I just deleted that Giant Egg entry. Difficulty is subjective, and the entry is causing more confusion than it's worth because it's not easy categorizable.

I left my other entry on Giant Egg providing easy infinite lives.

Depends on when. If It's early on, then we'll probably want to avoid life farms. Late in the game, we'll probably keep them in.

Do the world intro levels not yet have exits implemented? I've tried 1, 2, and 3 so far, and there seems to be no way out of them. Not sure if this is a "bug" or just something that's on the to-do list but not yet done.

Seems to me the boss battles would be significantly more challenging if they had longer invincibility frames. As is, their on-hit invincibility seem to be down around MM2 timing, which is significantly lower than most (all?) of the rest of the series.

Flare Man can also move around via high jump, either covering half the room or the entire room in one leap, with an earthquake stomp effect when he lands. That seems to be it.
